People with dementia and other forms of memory loss deserve ongoing excellent care for their unique needs. At Wesley Willows, we provide ability-centered memory care to help each person retain their dignity, skills, and independence. Seniors requiring ongoing assistance enjoy meaningful services and activities at our Kirk’s Place Residency or Day programs. We also offer purpose-driven activity program for residents noticing early signs of cognitive impairment at our Memory Wellness Center. It’s all part of the full continuum of care seniors receive at Wesley Willows.
No two seniors have the exact same memory care needs. For this reason, we provide flexible programs to support each of our residents with specialized care. Here are the different levels of care you can receive at our memory care facilities:
Residency at Kirk’s Place is ideal for seniors who have an official diagnosis of Alzheimer’s or other related dementia and need constant support. Residents receive specialized care and attention 24 hours a day while living in our secure and home-like facility.
Sometimes families are able to care for seniors with memory loss at their homes but need supervision during the work week. For this reason, we offer adult day care services at Kirk’s Place Monday through Friday from 9:00 am – 4:00 pm (excluding holidays).
Our independent living residents with early signs of memory loss find support at our Memory Wellness Center. The center is open Tuesdays and Thursdays from 10:00 am – 2:30 pm and offers purpose-driven activities for independence and cognitive health in an intimate group setting.
Our memory care professionals take a holistic approach to promoting resident self-esteem and mental health. Our caring team also encourages family and friends to share in the day-to-day experiences of their loved ones.
We strategically fill our activity calendars with both informal and formal structured activities to promote resident health from multiple dimensions of life enrichment, including:
Find spiritual fulfillment through chapel services, hymn sing-a-longs, Bible studies, devotionals, and one-on-one chaplain visits.
Keep active through morning stretches, outdoor walks, stress reduction exercises, group fitness sessions, and other activities.
Take comfort in a cozy, home-like setting with peaceful outdoor gardens and scenic bus trips to beautiful local attractions.
Enjoy a sense of purpose through service activities that support the local community, such as making baby blankets.
Get the emotional help you need through proven methods, such as music therapy, pet therapy, support groups, and more.
Keep your mind active and healthy through fun brain games, trivia, Montessori activities, cultural exploration, and more.
Our socially-supportive environment discourages withdrawal and isolation by facilitating community through fun events.
We provide meals and snacks that support a brain-healthy diet, which follows the Alzheimer’s Association’s recommendations.
Ample sensory-rich activities, crafts, and outings promote increased input and regulation for ongoing cognitive health.

We want to help you make the best decision for memory care. Here are answers to questions people typically ask:
A prospective Kirk’s Place resident or day program participant must be at least 55 years of age and have an official diagnosis of Alzheimer’s or another related dementia by a physician.
The pre-admission process for becoming a Kirk’s Place resident or day program participant begins with a phone or in-person interview with our Assistant Director of Memory Care or other qualified staff members. We will then discuss the timeline for admission with you. You are also welcome to schedule a tour of Kirk’s Place.
The program admission process also involves a financial, personal, and medical application. Our team will guide you through the entire process to keep it simple for you.
Wesley Willows residents who wish to participate in our Memory Wellness Center must be independent in all self-care tasks and needs. Residents must also be independently mobile and able to complete transfers on their own. Finally, they must be able to administer their own medication since no nursing services are provided at the Memory Wellness Center.
Rockford is one of the best cities for dementia care in the country. In December 2023, the city was officially recognized as the 28th Dementia Friendly Community in the nation, as part of the Dementia Friendly America movement. A Dementia Friendly Community makes a concerted effort to bring increased awareness, education, resources, experiences and strategies for inclusion to the general public. Wesley Willows is just one of several sectors of the Rockford community striving to make the region a more supportive and inclusive environment for people living with dementia and their loved ones.
Memories Through Music is a specialized approach to dementia care and is an example of the person-centered care that Wesley Willows embraces. Personalized playlists enable those struggling with dementia to reconnect with the world through memories triggered by the music they love.
Wesley Willows offers a Virtual Dementia Tour for staff and family members. This tour provides a glimpse of day-to-day struggles of living with dementia and an empathetic lens for staff to better understand what person-centered care entails.
We encourage all loved ones of residents at Kirk’s Place to visit as often as they wish and participate in activities with their loved ones. However, there is no one-size-fits-all answer for how often to visit people in memory care. The answer for you will depend on your relationship with your loved one.
If your loved one is used to weekly visits from you, then it will be good to maintain that pattern. However, you should also be mindful of the condition of their memory. For some seniors with more severe memory loss, visits can be overly stressful, which may mean that less frequent visits will be better for them.
If you’re unsure about how often to visit your loved one at our long-term care facility for dementia patients, we recommend consulting our staff. Our Assistant Director of Memory Care would be happy to help you determine the arrangement that makes the most sense for your loved one’s well-being.
At Wesley Willows, we use person-centered care to promote health and vitality for people with all types of dementia. We do this for everyone from independent living residents in the early stages of memory loss to people in need of constant supervision. Interested in our programs? We welcome you to schedule a tour of Kirk’s Place or our Memory Wellness Center to see what extraordinary memory care in Rockford is all about!
