Our Medicare-certified senior rehab center provides both inpatient and outpatient therapies under Medicare coverage. Usually, people will stay short-term to recover from injuries or illness and return to their homes. However, outpatient services are also available to Wesley Willows residents and those living in the Rockford community.


Residents who choose to make Wesley Willows their long-term home pay a daily fee for our skilled nursing lifestyle. We take into consideration long-term care, insurance benefits, and veterans’ benefits if applicable. Beyond this, the fees listed below are what you can expect to pay for your stay at our long-term care health center.


These fees vary based on the size of the room and the type of connecting restroom. Some have connecting private toilets while others have full private bathrooms.


The fees also differ for residents who have and have not paid entrance fees. An entrance fee is a one-time payment residents make when moving into Wesley Willows. Please call 815-316-1500 if you have questions about how the fee structure at Wesley Willows works.

  • What is a skilled nursing facility (SNF)?

    An SNF is a place where people receive short- or long-term care to help them recover from illnesses or injuries. This care can come from a wide variety of licensed medical professionals, such as registered nurses (RNs), nurse practitioners (NPs), certified nurses’ aides (CNAs), and more. SNFs offer 24/7 medical support for the ongoing health and rehabilitation of their residents.

  • Is a skilled nursing facility the same as a nursing home?

    No. While skilled nursing facilities and nursing homes provide similar services, they differ from one another in how long residents typically stay there. People usually stay at SNFs for a limited time to recover from illnesses or injuries and then return home once they recover. Conversely, a nursing home is a long-term assisted living facility where residents stay permanently when they need ongoing help with everyday activities. Wesley Willows provides both types of care on its campus.   

  • Are people who do not already live at Wesley Willows able to utilize the Medicare rehab services?

    Yes! Please work with your discharge planner or social worker to send a rehab referral to the team at Wesley Willows.

  • Should my loved one receive inpatient or outpatient senior rehab services?

    At Wesley Willows, our mission is to enable seniors to live as independently as possible. Therefore, we encourage residents to use outpatient services when they can. The question to ask to determine if outpatient rehab is a viable option is how much care does your loved one need for recovery? If your loved one can function independently with only minimal support needed, then outpatient rehab should suffice. However, we recommend inpatient rehab for seniors who need constant supervision, which is typically more than their family members can provide.

  • How long will Medicare cover my stay at Willows Health and Rehab Center?

    Medicare Part A limits coverage for a stay at a SNF like Willows Health and Rehab Center to 100 days. However, you must meet certain conditions to qualify for Medicare coverage, including a qualifying inpatient hospital stay and a doctor’s referral.
